Primary and preventative medical care services are provided by a team of highly trained and skilled physicians, nurse practitioners, pharmacists, registered nurses, and medical assistants. We strive to create a “medical home” for our patients with the goal of preventing the inappropriate use of the local emergency rooms for non-emergent situations. Through the Health Disparities Collaborative and using evidence-based practice, we designed a program destine to reduce disparities in health outcomes for minority and underserved people, we use the Chronic Care model. Through health education, this model empowers the patient to take charge in managing their own health. Additionally, Orange Blossom Family Health has onsite laboratory, vision and pharmacy services. Orange Blossom Family Health offers free rapid HIV testing in a confidential environment.

Patients who require a referral for conditions that are not managed by our primary care providers will be referred as per their insurance or through Orange County Secondary Services as applicable. Our referral coordinators assist patients in making appointments with a specialist, which can be challenging.
